La siderosis superficial del sistema nervioso central es un padecimiento muy infrecuente, caracterizado por sordera, síntomas cerebelosos y signos piramidales. Este desorden es consecuencia de hemorragia subaracnoidea crónica. La resonancia magnética de encéfalo muestra de forma característica, depósito marginal de hemosiderina alrededor de la superficie del sistema nervioso central en las secuencias ponderadas en T2. La forma más prometedora de tratamiento e la identificación y eliminación de la fuente de sangrado; sin embargo, una vez que la superficie del sistema nervioso central ha sido afectada de forma importante por hemosiderina, el hecho de eliminar la fuente de sangrado tendrá poco efecto en la progresión del deterioro clínico. Evitar retrasar el diagnóstico es muy importante en los casos susceptibles de tratamiento causal. Reportamos el caso de un paciente con desarrollo lento y progresivo de ataxia cerebelosa y pérdida auditiva debidos a siderosis superficial del sistema nervioso central.

Superficial siderosis of the central nervous system: Case report and literature review

[ABSTRACT]

Superficial siderosis of the central nervous system is a very infrequent disease characterized by deafness, cerebellar symptoms and pyramidal signs. This disorder is the consequence of chronic subarachnoid hemorrhage. Brain magnetic resonance imaging shows a characteristic marginal low T2-weighted signal of hemosiderin around the surface of the central nervous system. The most promising way of treating the disorder is to identify the source of bleeding and remove it; however, once the central nervous system surface is greatly affected by hemosiderin, even removing the source of bleeding will have little effect in the progression of clinical deterioration. Avoiding diagnostic delay is very important in those cases susceptible of causal treatment. We report the case of a patient with slowly progressive development of cerebellar ataxia and hearing loss due to superficial siderosis of the central nervous system.
