Eficacia del cierre percutáneo del foramen oval permeable en la prevención de eventos recurrentes comparado con el tratamiento médico estándar en pacientes con EVC isquémica criptogénica: Revisión sistemática y meta-análisis

Resumen

Introducción: Aún no es claro cuál es el tratamiento más efectivo para la prevención de un infarto cerebral o ataque isquémico transitorio (AIT) recurrente en pacientes con foramen oval permeable y enfermedad cerebral aguda (EVC) isquémica criptogénica.

Objetivo: Determinar la eficacia del cierre percutáneo del foramen oval permeable en la prevención de EVC isquémica recurrente en pacientes con EVC criptogénica.

Métodos: Se realizó la búsqueda en PubMed, EMBASE, KoreaMed, LILACS y The Cochrane Library. Se incluyeron ensayos clínicos aleatorizados y controlados que evaluaran la eficacia del cierre percutáneo del foramen oval permeable comparado con el tratamiento médico estándar para la prevención de EVC recurrente en adultos con EVC criptogénica. Se recalcularon los riesgos relativos [RR, e intervalo de confianza (IC) de 95%] para el desenlace primario de infarto cerebral fatal o no fatal, AIT omuerte por cualquier causa. El análisis combinatorio del tamaño del efecto se calculó con base en la población de intención a tratar (ITT) para el desenlace primario.

Resultados: De 108 referencias inicialmente identificadas, se incluyeron tres estudios que reunieron a satisfacción los criterios para este análisis: CLOSURE-I (n = 909), PC Trial (n = 980) y RESPECT (n = 414); publicados entre 2012 y 2013. El metaanálisis de los tres estudios fue sobre 2,303 pacientes del grupo de intención a tratar declarado en cada ensayo clínico (cierre percutáneo n = 1,150; tratamiento médico n = 1,153). El análisis combinado recalculado no demostró una reducción significativa del riesgo para el evento primario de infarto cerebral fatal o no fatal, AIT o muerte por cualquier causa (RR: 0.72; IC del 95%: 0.49-1.06). Este efecto no fue diferente con la exclusión secuencial de cada estudio.

Conclusiones: La mejor evidencia disponible muestra que el cierre percutáneo del foramen oval permeable no es superior al tratamiento médico estándar en la prevención de EVC isquémica recurrente o muerte en pacientes con EVC criptogénica.

Efficacy of percutaneous closure of patent foramen ovale in preventing recurrent events, compared with standard medical therapy in patients with cryptogenic ischemic stroke: Systematic review and meta-analysis
Abstract
INTRODUCTION: It is not clear what is the most effective treatment for the prevention of recurrent stroke or transient ischemic attack (TIA) in patients with patent foramen ovale and cryptogenic ischemic cerebrovascular disease (CVD).

OBJECTIVE: To determine the efficacy of the percutaneous closure of patent foramen ovale in the prevention of recurrent stroke in patients with cryptogenic stroke or TIA.

METHODS: We performed a search in PubMed, EMBASE, KoreaMed, LILACS, and the Cochrane Library databases. We included randomized controlled trials evaluating the efficacy of percutaneous closure of patent foramen ovale compared with standard medical therapy for the prevention of recurrent stroke in adults with cryptogenic CVD. Were recalculated relative risks [RR, and 95% confidence interval (CI)] for the primary outcome of fatal or non-fatal stroke, TIA or all-cause mortality. The combined analysis of the effect size was calculated based on the intention-to-treat population (ITT) for the primary outcome.

RESULTS: Of 108 references initially identified, we included 3 studies that satisfactorily met the criteria for this analysis: CLOSURE-I (n=909), PC Trial (n=980) and RESPECT (n=414), published between 2012 and 2013. The meta-analysis of the 3 studies was on 2,303 patients in the intention-to-treat populations of each study (percutaneous closure n=1,150; n=1,153 medical treatment). The combined analysis recalculated showed no significant reduction of the risk for the primary endpoint of fatal or non-fatal stroke, TIA or all-cause mortality (RR: 0.72; 95% CI: 0.49-1.06). This effect was not different with the sequential exclusion of each study.

CONCLUSION: The best available evidence to date shows that percutaneous closure of patent foramen ovale is not superior to standard medical therapy in preventing recurrent stroke or death in patients with cryptogenic acute ischemic CVD.
