RESUMEN

INTRODUCCIÓN: Los germinomas del SNC son tumores primarios que se localizan en región pineal, compartimiento supraselar y III ventrículo. Generan trastornos hipotalámicos e hipofisiarios, alteraciones endocrinas, trastornos conductuales y hemianopsia. Se tratan con cirugía, radioterapia y/o quimioterapia. Lo que favorece alteraciones morfológicas y/o funcionales en el SNC, entre las que se encuentra deterioro neuropsicológico (enlentecer el procesamiento cognitivo, declive en CI, déficit en atención, habilidades motoras, memoria, fluidez verbal, procesamiento secuencial y habilidades académicas).

CASO CLÍNICO: Se evaluó a un paciente con germinoma multicéntrico en región selar; se aplicó NEUROPSI, WISC IV, TAVECI, figura de Rey test de Boston, D2, test de los 5 dígitos, torre de Londres y test de clasificación de tarjetas de WISCONSIN). El paciente presentó alteraciones en proceso y control atencional, memoria, lenguaje, aprendizaje y funciones ejecutivas (planeación, organización, inhibición, automonitoreo, velocidad de procesamiento).

CONCLUSIÓN: Las alteraciones son consistentes con disfunciones en tálamo, circuito fronto-temporal, corteza frontal dorsolateral y giro angular.

ABSTRACT

Neuropsychological profile in a case of multicenter germinoma

INTRODUCTION:  CNS germinomas are primary tumors located in the pineal region, suprasellar compartment and third ventricle. They cause pituitary and hypothalamic endocrine alterations, behavioral disorders and hemianopia. These tumors are treated with surgery, radiotherapy and/or chemotherapy, which favor morphological and/or functional changes in the CNS, including neuropsychological impairment (slow cognitive processing, IQ decline, attention deficits, altered motor skills, memory, verbal fluency, sequential processing, and academic skills).

CASE REPORTS: A patient with multicenter germinoma in the sellar region was evaluated. A test searching for alterations in most cognitive functions (NEUROPSI) was applied, also a general test of intelligence an IQ (WISC-IV), and specific tests to evaluate memory (TAVECI and REY), language (Boston test), and executive functions (D2, 5 digits test, tower of London and classification test of WISCONSIN cards). The patient presented disturbances in the attention control process, memory, language, learning process and executive function (planning, organization, inhibition, self monitoring, and processing speed).

CONCLUSION: The disturbances are consistent with thalamic dysfunctions, front-temporal circuit, frontal dorso-lateral cortex and angular gyrus.
